Mobility aids such as wheelchairs and walkers need to be assessed and provided through the NHS Community Occupational Therapy and Physiotherapy to access these services you should contact your G.P to begin this process.

If you are looking for the provision of a mobility scooter this would be required to be purchased privately and Sefton Council are unable to assess or advise for use or suitability of items of this type.

There is a range of questions about things such as washing and dressing, how you move around your home and getting out and about.

It should take no longer than 20 minutes to complete but you can take as long as you like and you can also save it and return later if you wish.

Answering the questions will help us provide you with some information and links to opportunities in the community which you may find useful.

The form, if submitted at the end, is an initial contact with Sefton's Occupational Therapy team.
